How often will I need to have “touch up” or “maintenance” breast procedures?

There is no touch up or maintenance necessary, although there is a deflation rate of 3-7%. If this occurs in the first 10 years, there is no charge (for our patients) to replace the implant. Otherwise, the implants may last indefinitely. When contemplating this surgery, one should enter into this with the understanding that at some point in their life they may want or need an additional surgery.

Is it better to place a breast implant above or below the muscle?

It is much better for most patients to place the implant below the muscle as this results in a more natural appearance and has a much lower rate of capsular contraction, or hardening.
For the patient with one breast very underdeveloped or tuberous in shape, the implant may need to be placed over the muscle. Also if a patient has had previous augmentation over the muscle it is nearly impossible to replace under the muscle.

Is it possible to take fat from my buttocks to fill my breasts?

Yes, fat may be harvested from multiple sites and placed in the breast (or face, hands, other depressions).
The final size is not as predictable as with implants and a touch up is usually required. Not all the fat survives, but what does is permanent and there are no worries about leaks or replacement or permanent hardness of the breast. Not very many patients qualify for this procedure, but for those who do it can be a good alternative. The costs are comparable to the implant procedure.
